• For Dental Hygiene Services — $55 per hour
• For Dental Services — $70 per hour
• G.P.A fee for Referred Personnel — $62/placement
• G.P.A. fee for Permanent Placement — $800/per day of the week
• Continuing Care Perio Documentation Forms — $65/200 sheets
• Personal Coaching — $100 per hour either in person or on the phone
